How can I add a new Member to my Membership?

To add a new Member to your Membership, notify Solidarity HealthShare and complete the correct form, as listed below. Please complete one form for each additional member.

Type of Member addition:

  1. Adding a spouse or dependent child – submit Add a Member: Medical History Questionnaire. This form must be submitted by the first day of the month prior to the month in which the Member intends for the change to take effect. 
  2. Adding a newborn – submit Newborn Enrollment Form. Your newborn will be added to your existing Membership as of the date of their birth if this form is received within thirty (30) days of their birth date. If you have missed this deadline, please complete the Add a Member: Medical History Questionnaire mentioned above. Your newborn will be added to your Membership effective the first day of the next month according to our Sharing Guidelines.

Adding a member will be subject to the application process (e.g., medical history review, 60-day wait period on preventative care, pre-existing conditions, and conditional membership). Visit our Sharing Guidelines for more details.

Please Note: Adding a new Member to your membership may result in a change to your Annual Unshared Amount (AUA) and/or your Monthly Contribution.
