When it comes to managing your Membership, filling out the right form is key. Submitting the wrong form can lead to delays and frustration. To help make this process smoother, we’ve outlined what each Membership change form is used for—so you can make changes with confidence!
🧾 Understanding the Most Common Forms
Below is a breakdown of the most frequently used forms, so you can quickly identify the one that best fits your needs:
➕ Add A Member Form

Use this if: You want to add a spouse or dependent to your Membership.
- To add a newborn to your Membership within thirty (30) days of their birth, please fill out the Newborn Enrollment Form.
- To add a newborn to your Membership thirty (30) days after their birth, please complete the Add a Member form.
Each new person needs their own form and a completed Medical History Questionnaire.
❌ Member Exclusion Form
- Use this if: You need to remove a spouse or dependent from your Membership

If you're removing a spouse, their written consent is required.

🔄 Primary Member Change or Roll-Off Form

Use this if: You want to change the Primary Member on the Membership or if you want to move someone to their own Membership
- There’s no fee or waiting period for this change but some other rules do apply.
🔁 Program Change Form

Use this if: You want to switch to a different Membership program (such as adjusting your AUA or switching from Legacy Premier to Solidarity One).
- You can increase your AUA one time during your current Membership year. However, decreasing your AUA is only permitted during your renewal period (beginning on the 1st of the month prior to your renewal date).
- If you switch programs from Legacy Premier to Solidarity One, you will not be able to revert back to Legacy Premier.

- Review the form title and description carefully before you begin.
- Contact the Service Experience Team if anything is unclear or doesn’t seem to fit your situation.
