If you need to add a Member to your Membership, you can do so by filling out this form: Add a Member: Medical History Questionnaire.
A few quick notes before you get started:
- This form is for an individual applicant. You will need to fill out the form for each Member you would like to add to your Membership.
- Please note that the Medical History Questionnaire asks detailed questions. If you need additional time to gather the necessary details, you are able to save your Add A Member Form and return at a later time. You will be sent an email with a link to return to the Medical History Questionnaire. At the bottom of each page of the form, there is a button to "Save and Resume Later." If you decide to use this feature, you have 30 days to return to complete your form and must use the link provided to retrieve your progress.
- If you are trying to add a newborn to your Membership within 30 days of their birth, please instead email the Maternity coordinator that worked with you during your pregnancy and they will be able to help you add your child to your Membership.
